Authorization terms
[This is a verbal authotization form authorizing Hialeah City FC to charge fisted above.] the above credit card for any payment and small service for the amount submitted on the selected date. Each time the card is declined there will be a $20 fee.
Terms and Conditions
These Terms and Conditions apply to Hialeah City Football Club (“Hialeah City F.C.”, “the Club”, “we”, “us”) and all domain names, websites, and digital platforms associated with the Club.
By registering, enrolling, making a payment, or checking the acceptance box, the Participant and, if applicable, the Parent(s) or Legal Guardian(s), acknowledge that they have read, understood, and agreed to be bound by the following Terms and Conditions.
1. Programs and Services
Hialeah City F.C. offers youth soccer programs and related activities, including but not limited to:

Soccer School
-
Soccer Camps
-
Soccer Clinics
-
Competitive and recreational teams
-
Training sessions, tournaments, and events
-
2. Registration
Registration is subject to availability and becomes effective only after:

Full payment has been received and cleared, and
-
Registration has been confirmed by Hialeah City F.C.
A completed registration form and payment of all applicable fees are required to reserve a space.
3. Participant Medical Conditions
Participants with chronic medical conditions (including but not limited to diabetes, seizure disorders, severe allergies, or mental health conditions) must contact the Club at (786) 651-1423 prior to registering.
The Participant and Parent(s) or Guardian(s) certify that the Participant does not have any condition that would pose a safety risk and agree to disclose any condition requiring medical attention or accommodation.
Hialeah City F.C. does not guarantee the availability of special accommodations.
4. Competitive Team (Blue Team) Tournament Commitment
Participants selected for Competitive (“Blue”) Teams must:

Participate in a minimum of four (4) official tournaments per year

Cover all tournament-related expenses (travel, lodging, meals)
Failure to comply without approved justification may result in reassignment to a recreational team without refund or credit.
5. Pricing
All prices are listed in U.S. Dollars and are subject to change without notice.
No refunds or adjustments apply to completed transactions.
6. Payment Policy (Non-Payment)
All fees must be paid according to the Club’s payment schedule.

Payments more than 10 days late may result in suspension from all activities

Payments more than 30 days late may result in termination of enrollment

All balances must be paid in full before returning

No refunds or credits will be issued for missed sessions due to non-payment
7. Assumption of Risk & Waiver of Liability
Soccer is a physically demanding activity involving inherent risks.
The Participant and Parent(s) or Guardian(s) voluntarily assume all risks and release Hialeah City F.C., its staff, volunteers, and affiliates from any claims, including negligence, arising from participation.
The Participant confirms they are physically and mentally capable of participation.
8. Weapons & Prohibited Items
Possession of firearms, knives, weapons, or weapon-like objects is strictly prohibited and will result in immediate expulsion without refund.
9. Illegal Substances
Possession or use of illegal drugs, alcohol, or prohibited substances will result in immediate expulsion and possible referral to law enforcement. No refunds will be issued.
10. Program suspension and force majeure.
Hialeah City F.C. may cancel training sessions due to weather, force majeure, facility issues, or other circumstances beyond its control. These sessions will not be rescheduled.
11. Rain and adverse weather policy.
Programs may be canceled for safety reasons. Not recoverable.
12. Photo & Media Release
Participants grant permission for Hialeah City F.C. to use photos and videos for promotional purposes for up to two (2) years following registration.
13. Personal Property & Damages
The Club is not responsible for lost or stolen items.
Participants are responsible for damages caused to facilities or equipment.
The credit card on file may be charged for unpaid balances or damages.
14. Player Release (Transfers)
Release requests must be submitted by certified letter to:

A $500 release fee, payable in cash, applies.
15. Cancellations & Refunds

Club Programs: All fees are non-refundable

Camps / Clinics:

$100 fee if canceled more than 30 days prior

No refunds within 30 days of start date

16. Arbitration
Disputes not resolved through negotiation shall be settled by binding arbitration under the rules of the American Arbitration Association near Miami, Florida.
17. Right of Admission Reserved
The Club reserves the right to deny admission for disruptive, unsafe, discriminatory, or rule-violating behavior.
18. Responsible Data Use & Cookies Policy
Hialeah City F.C. is committed to the responsible and transparent use of personal data collected through its websites, registration systems, and digital platforms.

Personal information is collected solely for legitimate club operations, including:
